A hospital discharge in Grand Rapids

The first 48 hours, and the questions that decide the next three months.

Quick answer

Ask the case manager three things the same day: whether the plan is home with services, short-term rehab or long-term placement; whether the stay qualifies for Medicare rehab coverage; and which facilities or agencies have accepted the referral.

What should a family do when a Grand Rapids hospital says a parent is ready for discharge?

Ask the case manager three things the same day: whether the plan is home with services, short-term rehab or long-term placement; whether the stay qualifies for Medicare rehab coverage; and which facilities or agencies have accepted the referral.

Discharges from Corewell Health Butterworth or Blodgett, Trinity Health Grand Rapids, University of Michigan Health-West, Holland Hospital and the other area hospitals move fast, often within a day or two of the decision. You are allowed to choose among facilities and agencies that accept your parent; the hospital's list is a starting point, not an order.

Does Medicare pay for short-term rehab after a hospital stay?

Often. Original Medicare covers skilled nursing facility care after a qualifying inpatient hospital stay of at least three days when daily skilled care is needed, with no coinsurance for days 1-20 and a daily coinsurance after that, up to 100 days.

Watch for observation status: days in the hospital under observation do not count toward the three inpatient days. Ask the case manager in writing whether your parent was admitted as an inpatient. Medicare Advantage plans set their own rules and often require prior authorization.

How do you choose a rehab facility in Grand Rapids quickly?

Start with the homes that accepted the referral, check each one's Care Compare stars and latest citations, and favor strong staffing; of the 49 certified homes in our area, 35 have a staffing rating of 4 or 5 stars.

Our nursing home directory sorts by rating and shows each home's latest inspection. Hospital-affiliated rehab units near the Medical Mile tend to rate well but fill quickly.

What if a parent cannot go home after rehab?

Use the rehab stay to plan: tour assisted living or AFC homes while your parent is still covered, ask the rehab team for a frank assessment of care needs, and start any Medicaid or VA application early.

MI Choice can help if your parent qualifies for nursing-home-level care but wants to be at home; Michigan Medicaid can cover long-term nursing home care once countable assets are at or below $9,950 (2026, BEM 400).

What should you bring to the discharge planning meeting?

A list of medications, the names of your parent's doctors, insurance cards, any power of attorney or patient advocate papers, and a realistic picture of who can help at home and when.

Ask for the discharge plan in writing, including home health orders and equipment. If you disagree with the plan, Medicare patients can request a review of the discharge decision -- ask the case manager for the notice that explains how.
